At every second scheduled dry-docking (i. e. every 5 years, usually) the Special Survey for renewal of class will be carried out (leading to the issue of a new Certificate of Class).

Yes, if the ship is operating at a higher security level than the port facility it intends to interface with.

(1) That the ship is equipped with a sewage treatment plant or comminuter or holding tank and a discharge pipeline, as described in the certificate; (2) that the ship has been surveyed; and (3) that the survey shows that the ship's structure, equipment, systems, fittings, arrangements and material are satisfactory and that the ship complies with MARPOL Annex IV.
