A form entitled "Amendments to the Continuous Synopsis Record (CSR) Document Number... for the Ship with IMO Number ......", also called a CSR Amendment form. It is completed by the Company or the master when any detail recorded on the CSR Document (Form 1) changes, and must be attached to that CSR Document. A copy must then be sent to the flag State Administration, who will then (normally) issue a new CSR Document (Form 1).
