What document is required by SOLAS to supplement a Cargo Ship Safety Radio Certificate?

(1) particulars of ship (name, call sign and number of persons with required qualifications to operate the radio installations); (2) details of radio facilities (actual provision of VHF and MF R/T, DSC, Navtex, EGC, Inmarsat SES, etc. ); and (3) methods used to ensure availability of radio facilities (duplication of equipment, shore-based maintenance or at-sea maintenance capability).
