What document can a seaman request from the master, even after he has left the ship?

A certificate (separate from any other document) either as to the quality of his work or indicating whether he has fully discharged his obligations under his contract of employment. The master, or another authorised officer, must give him this, if the seaman asks for it, within 6 months from the date of his discharge from or his leaving the ship.
